How to find a Dentist

Culinary Network Dentists may change. For an updated list or to find a dentist in your area:

Call 702-790-8000 to make a dental appointment at the Culinary Health Center or
Call Nevada Dental Benefits at 702-478-2014 or
Go to Nevada Dental Benefits’ website and in the Find A Dentist section choose NDB Managed Care Dental Plans

Not all dentists who say “Culinary is accepted here” are in our network! Please call NDB at 702-478-2014 to make sure your dentist is in the network. This helps you avoid extra costs.

Missed appointments

It is important for you to call your dentist at least one day before your appointment if you have to cancel. The Dental Plan will not pay any fee for missed appointments.

Dental emergencies

If you have a dental emergency, call your dentist. If your dentist is unable to see you within 24 hours, contact NDB at 702-478-2014.

Benefits at an IN-NETWORK Dentist

Deductible: None
Maximum Yearly Benefit: No Maximum
Copays: Depends on the service

Dental services must be from a Culinary Network dentist in the Nevada Dental Benefits (NDB) group.

Your copays will depend on the service that you get. There are no copays for:

Exams
Basic cleanings
X-rays
Silver fillings

Dual Culinary Coverage

If you and your spouse both have CHFtoo benefits, the secondary CHFtoo coverage may cover a portion of the orthodontic copay only.

Benefits at an OUT-OF-NETWORK Dentist

There are no out-of-network benefits for CHFtoo participants.

You must see an in-network dentist.

Orthodontic benefit (Braces)

Dental services must be from a dentist in the network.

For dependent children under the age of 19:

Length of treatment: 24 months
Copays: $2,000
- $400 at the beginning of treatment
- $125 per month for 11 months
- $225 for retention (or for retainers)

Ages 19 and over:

CHFtoo doesn’t have adult orthodontic services for ages 19 and over.

You will only receive your orthodontic benefit during the months that you are eligible and in treatment.
If you and/or your eligible dependent already have braces and are new to the plan, CHFtoo will not cover your treatment.
