Our Diabetes Passport program gives you rewards for managing your diabetes. The program is easy to complete. It’s designed to be part of what you should do regularly to control your diabetes.
How does the program work?
Complete all 5 steps and get rewards along the way. It’s just 3 classes and 2 checkups.
Get a foot check (make an appointment with a podiatrist)
Get an eye check (make an appointment with an ophthalmologist or retina specialist).
As you complete the program, you get:
A blood sugar monitor
A portion plate
Measuring cups and spoons
A fruit infusion water bottle
Portion control containers
